A new, illustrated leaflet about holistic needs assessment (HNA). Developed as part of an improving personalised care and support research study, it has been co-designed with patients and staff from Guy’s & St Thomas’ NHS Foundation Trust, Northern Care Alliance NHS Foundation Trust, Imperial College Healthcare NHS Trust and Macmillan Cancer Support. The leaflet provides a simple, accessible overview of what an HNA involves and how the resulting support plan can help people identify and address their needs.

It may be particularly useful when first offering an HNA, or for people who would benefit from a brief introduction before completing one. It can also be used alongside Macmillan’s more detailed HNA booklet, which provides further information about planning care and support
